This form is for New Clients who are coming to our clinic for the first time. It is basic information about you and your cat.
Boarding
This form is for when you are bringing your cat in for boarding. The first page is a consent form with questions about their stay, the second form is mostly for the care team to track how your cat is doing through their stay.
Drop Off History
This form tells us what is going on with your cat, as well as valuable information about their daily routine and health. This form should be paired with the Drop Off Treatment Consent Form.
Drop Off Treatment Consent
This form should accompany the Drop Off History Form. This form gives us consent to treat your cat as well as goes over treatment options for their upcoming drop off visit.
